During his leadership, Dr. Mackey guided the National Registry through a period of significant transformation and advancement. His tenure saw the modernization of examinations across all certification levels, including the launch of the new AEMT and Paramedic examinations and the retirement of the ALS Psychomotor Examination. He also oversaw the launch of the Longitudinal Inquiry into Fire and EMS Health (LIFE) Study, a landmark national research initiative examining long-term occupational health risks among firefighters and EMS Clinicians.
These milestones reflect Dr. Mackey’s steadfast commitment to strengthening EMS certification, advancing professional competency, and ensuring that every Clinician is prepared to deliver the highest standard of prehospital care.

Dr. Mackey serves as Medical Director for Sacramento Regional Fire Services in Sacramento, California, and brings more than two decades of experience in EMS—beginning as a Clinician and now serving as Regional Medical Director and Fire Service Medical Director for the second-largest ambulance provider in California. He earned his Doctor of Medicine from St. Louis University School of Medicine, completed his Emergency Medicine Residency at the University of Pittsburgh, and holds both a Master’s and Bachelor’s degree in Biology from California State University.
Beginning January 1, 2026, Dr. W. Scott Gilmore will assume the position of Board Chair, and Dr. Mike McEvoy will transition into the role of Immediate Past Board Chair.
